The Child Health Report must be updated regularly. The form must be completed and signed by the your child's doctor. Please take a copy of this form to your child's well-check visit.

This form must be completed in full in order to authorize administration of medication in school. Please note that the medication must be in it's original child-safe container and labeled by the pharmacy with the child's name. Dosage and storage instructions must be clearly labeled.

This form is the first place we go to in the case of an emergency. Please update the form in the case of moving, new phone numbers or changes of whom we should contact in the case of an emergency. 

If your child has an allergy listed on their application or on their medical form, an allergy plan signed by their doctor will be required.
